Established Patient Visit
$125
first 15 minutes
Reserve focused time to review your progress, current concerns, or ongoing care plan. Additional time may be scheduled in 15-minute blocks for $125 per block.
Appointment fees cover provider consultation time. Laboratory testing, medications, procedures, and other services are billed separately unless included in a selected care program.
Self-Pay Visit Questions
Do I need a membership to be seen?
No. You can book a self-pay visit and pay per visit. If you start an ongoing program, you can choose a membership later.
Is there a booking deposit?
No. There is no deposit to book. Visit fees are charged for the time reserved with your provider.
What if I need to cancel or reschedule?
Please give at least 24 hours’ notice. Missed appointments and cancellations with less than 24 hours’ notice are $100 for new patients and $50 for established patients, except in a true emergency.
Can I use insurance for my visit instead?
If you have an eligible PPO plan or Medicare, see the Insurance tab. Our team can review your benefits before your visit.
Can I have a telehealth visit?
Yes. Visits are available in person at our Pasadena office or by video for patients located in California, Florida or Texas at the time of the visit.
Are labs and medications included in the visit fee?
No. Visit fees cover your time with the provider. Lab tests, medications and procedures are billed separately, and our team will explain costs before they're ordered.
What if my visit needs more time?
If more time is needed, an additional 15-minute block is $125.

Membership Questions
What does my monthly fee include?
Your selected program’s visits, monitoring labs, prescribed program medications, supplies and standard shipping. With a PPO insurance membership, visits and labs are billed to your plan instead. Other tests or services are billed separately and explained before they’re ordered.
How often will I be seen?
Your first month may include two to three visits to evaluate, review results and plan treatment. After that, self-pay memberships include a 15-minute follow-up each month. PPO insurance members need a visit at least once every three months.
Which labs are included?
Included labs are the monitoring labs for your selected program. Any other tests are billed separately and explained before they’re ordered.
Can I switch between self-pay and PPO insurance pricing?
Yes. If your coverage changes or ends, let us know. If you no longer qualify for PPO insurance pricing, you can switch to a self-pay membership or cancel.
How do medication refills work?
Refills are not automatic. Request them through your patient portal or secure messaging before your supply runs out. Monthly billing doesn’t always mean a new shipment every month; timing depends on your dose and monitoring.
Does membership cover my copay or deductible?
No. Membership is not insurance. With a PPO insurance membership, your plan's copays, coinsurance and deductibles still apply to visits and labs billed to insurance.
How do I cancel my membership?
Monthly memberships have no long-term commitment. To cancel, send written notice at least 30 days in advance, as stated in your financial agreement, through your patient messaging channel or by email to billing@alphahormones.com. Annual memberships are prepaid and non-refundable: if you cancel during the year, unused months are not refunded or prorated.

Insurance Questions
What will I owe?
Your plan’s copays, coinsurance and deductibles. Services your plan doesn’t cover are billed at self-pay rates, and we’ll tell you in advance when possible.
Are lab tests billed to insurance?
Lab tests may be billed to your plan. Coverage depends on your plan and the tests ordered, so ask our team before your blood draw if you’re unsure.
Does insurance cover my medications?
It depends on your plan and the medication. Some prescriptions, including compounded medications and peptides, are often self-pay. Our team will let you know before you start.
Can I use insurance with a membership?
Yes. Patients with an in-network commercial PPO plan can choose a PPO insurance membership at $50 per month less than self-pay. It isn’t available with out-of-network plans or Medicare. See the Membership tab for details.
How will I hear back about my benefits?
After you submit the form, our team reviews your eligibility and benefits and replies by text or email. If you haven't heard from us within one business day, call or text (213) 334-4111 to check on the status.
What if I have secondary insurance?
Include any secondary or supplemental coverage when you submit your insurance card. Our team will review it along with your primary plan.
What if my insurance changes?
Let us know as soon as possible and submit your new card so we can verify your benefits before your next visit. If you have a PPO insurance membership and no longer qualify, you can switch to a self-pay membership or cancel.
