Stay Connected to Your Health Between Visits

Remote patient monitoring may allow eligible patients to share selected health measurements from compatible connected devices. When incorporated into a care plan, this information can help your medical team follow trends, support ongoing communication, and make more informed decisions about your care.

Eligibility, monitored measurements, devices, and review frequency depend on the individual care plan. Remote monitoring does not replace scheduled medical visits or emergency services.

Common Questions

Remote Patient Monitoring FAQs

What is remote patient monitoring (RPM)?

RPM lets eligible patients share readings such as blood pressure, weight or blood sugar from connected devices with their care team. Your team reviews trends between visits and reaches out when your care plan needs attention.

Who is eligible?

RPM is generally for patients with an ongoing condition that benefits from regular monitoring, such as high blood pressure or diabetes. Your provider decides whether RPM fits your care plan.

What devices are used?

Common devices include cellular-connected blood pressure cuffs, scales and glucose meters. Your care team will explain which device you need and how to use it.

Is my health data secure?

Readings are sent to a secure monitoring platform used by your care team. Only the information needed for your care plan is collected.

Does RPM replace office visits?

No. RPM supports, but does not replace, scheduled visits. It is not an emergency service. For a medical emergency, call 911.

Does insurance cover RPM?

Medicare and many commercial plans cover RPM for eligible patients. Copays, coinsurance or deductibles may apply. Our team can review your coverage before you enroll.
