This purchase policy is a binding agreement between Alpha Hormones Inc. (“Alpha Hormones,” “Alpha,” “we,” “us,” or “our”) and the person using our medical services (“you,” “your,” “patient,” or “client”). By making a purchase, you accept all the terms below for your personalized therapy.
Your monthly hormone subscription begins on the date of your first consultation. Each month after that includes one 15-minute telehealth or in-person consultation, along with the medications and lab tests specified for your package.
The included blood tests are CMP and IGF-1.
You agree to pay out of pocket for extra diagnostic tests and medications that your subscription or insurance does not cover. These may include specialized hormone saliva and urine tests.
All purchases are final. Our medical services include diagnostic tests and medications prepared for individual patients. Once we package them for you, we cannot use them for another patient.
Our affiliated pharmacy’s regulations prohibit returns of prescription medications for reimbursement. Alpha Hormones and the pharmacy cannot accept treatment returns for a refund, even if the treatment is unused.
We are not responsible for lost, stolen, or discarded packages that you fail to pick up. If we issue a refund, you are responsible for the debit or credit card processing fee on the total refund amount. We reserve the right to apply a no-refund, no-return policy to all purchases.
By purchasing a monthly subscription, you expressly agree that:
We disclose your recurring monthly charges during the purchase process.
To cancel, notify Alpha Hormones Inc. in writing at least 30 days before your next billing date. Email requests are acceptable. If a payment date falls on a weekend or holiday, we will process payment on the next business day.
There is no prorated refund for the remaining days of your subscription.
You must contact Alpha Hormones to request each medication refill. We do not refill prescriptions automatically. Refill timing depends on your dosage, rather than a fixed 30-day period.
We recommend requesting refills through your patient portal or Klara, our secure HIPAA-compliant text messaging platform.
You confirm that:
We will refer unpaid balances to a collection agency. You agree to pay all fees associated with collecting a delinquent account or enforcing this agreement.
If you have insurance, you remain financially responsible for other covered services provided by Alpha Hormones. We will bill you or your insurer for those services, as applicable.
You are responsible for all copayments, coinsurance, and deductibles required by your insurance coverage.
As a courtesy, we may file an insurance claim for services provided outside your covered subscription services or personal services. In these cases, you are responsible for an office visit charge.
Any dispute or claim related to your use of Alpha Hormones services will be resolved through binding arbitration or litigation in California.
Alpha Hormones Inc. cannot be held liable for interruptions to its services. We reserve the right to terminate services or subscriptions at any time.
