This purchase policy creates a binding agreement between Alpha Hormones Inc. (“Alpha Hormones”, ”Alpha”, “we”, “us”, or “our”), and the person (“you“, “yourself“, “your“, “patient”, or “client”) using our medical services. With your purchase, you accept the below terms and conditions in full for your personalized therapy.
If you purchase a monthly hormone package, the subscription will start on the date of your first consultation. Every month after includes one 15-minutes telehealth or in-person consultation with specific medication and labs. The included blood lab panel for Men’s Testosterone Replacement Therapy (TRT) package contain CBC, CMP, Estradiol, SHBG, and Testosterone, Free, Direct with Total Testosterone. The medications in our men’s TRT package include testosterone and if needed aromatase inhibitor such as anastrozole. HCG is not included. The included blood lab panel for Women’s Bioidentical Hormone Replacement Therapy (BHRT) package contains CBC, CMP, DHEA-S, Estrone, Estradiol, Estriol, FSH, LH, Progesterone, SHBG, and Testosterone, Free, Direct with Total Testosterone. The medications in our women’s BHRT package include estrogen, progesterone, and testosterone. DHEA is not included and can be purchased over the counter. For Human Growth Hormone (HGH) peptide therapy package, the blood lab panel includes CMP and IGF-1. You also agree to pay the cost of any extra diagnostic tests (i.e., specialized hormone saliva and urine tests) and medications cost out of pocket if it’s not covered by your medical subscription or insurance. Because our medical services include diagnostic tests and medications, so once they are packaged for each patient they cannot be used for another patient. Therefore, all purchases are final. Our affiliated pharmacy’s regulations prohibit the return of prescription medications for reimbursement. This means that Alpha Hormones and affiliated pharmacy are unable to accept returns of treatment for refunds, even if it is unused. Moreover, we are not responsible for lost, stolen, and discarded packages that you’ve failed to pick up. If a refund is issued by us, you are subject to the debit/credit card processing fee of the total refund amount. Ultimately, we reserve the right to hold a no refund and no return policy on all purchases made.
When you purchase a monthly subscription, you expressly acknowledge and agree that [1] Alpha Hormones (or a third-party payment processor) is authorized to charge you on a monthly basis for the price of your chosen subscription (in addition to any other charges set forth during the purchase process) for as long as your subscription continues, and [2] your subscription is continuous until you cancel it or we suspend or terminate the subscription. The monthly recurring charges for your subscription will be set forth during the purchase process. Should you become dissatisfied with your subscription and wish to cancel it, please notify Alpha Hormones Inc. in writing of your termination request at least 30 days prior to the next billing date. If the payment date falls on a weekend or holiday, the payment shall be executed on the next business day. Email requests are acceptable. You understand there is no “pro-rated” refund for the remaining days.
Patient is required to contact Alpha Hormones to request any and all medication refills. We will NOT automatically refill prescriptions. Your refill is based on your dosage not 30 days time period. We recommend using your patient portal or Klara, our secure HIPAA compliant text messaging platform, to contact your medical provider for your medication refills.
Should you become dissatisfied with your subscription and wish to cancel it, please notify Alpha Hormones Inc. in writing of your termination request at least 30 days prior to the next billing date. If the payment date falls on a weekend or holiday, the payment shall be executed on the next business day. Email requests are acceptable. You understand there is no “pro-rated” refund for the remaining days.
You represent and warrant that [1] any payment information you supply is accurate, true, and complete, [2] any charges incurred for medical services will be honored by your debit/credit card company or bank, [3] you will pay all charges incurred by you, and [4] the payment card is in your name and you are authorized to use such card for purchases. Unpaid balance will be referred to a collection agency. Patient agrees to pay all fees associated with the collection of the delinquent account under this agreement or otherwise incurred in enforcing the terms of this agreement. If you have insurance, you shall be financially liable for other covered services provided by Alpha Hormones and you or your insurer, as the case may be, will be billed for these other services. You remain financially responsible for all co-payments, co-insurance and/or deductibles as defined by the terms of your insurance coverage for provision of covered services. Moreover, we can file a claim with your insurance company as a courtesy only with respect to services that are provided other than as part of the covered subscription services or as personal services. Under such circumstances, you will be responsible for an of office visit charge. Any dispute or claim relating in any way to your use of Alpha Hormones services will be resolved by binding arbitration or litigation in the state of California. Alpha Hormones Inc. cannot be held liable for any interruptions of the provided services and reserves the right to terminate services/subscription at any time.
