Testosterone is an androgen involved in sexual function, sperm production, red-blood-cell production, bone health, muscle mass, and other important physiological processes.
Testosterone levels may change with age, health conditions, medications, sleep, body composition, and other factors. A low laboratory result by itself does not establish a diagnosis of hypogonadism.
Diagnosis requires compatible symptoms together with consistently low testosterone levels measured using reliable laboratory testing.
Symptoms alone are not enough to diagnose testosterone deficiency. Laboratory testing is needed to determine whether testosterone is consistently low.
When treatment is clinically appropriate, the formulation should be selected according to health history, preferences, cost, fertility plans, treatment goals, and monitoring needs.

Gels and other topical formulations are applied to the skin. Patients should follow precautions intended to prevent accidental transfer to partners, children, or other household members.
Oral testosterone undecanoate capsules are an FDA-approved option for eligible men. They are absorbed primarily through the intestinal lymphatic system, reducing first-pass liver metabolism. This might be a better option if a patient has elevated hemoglobin and hematocrit because of its short half-life.
Depending on clinical circumstances and availability, options may include subcutaneous testosterone pellets, patches, nasal preparations, buccal systems, or longer-acting injections. Each method has distinct dosing, safety, convenience, and monitoring considerations.
The medications below are separate from standard testosterone formulations. They may be considered only for selected clinical goals, diagnoses, and laboratory findings.
Enclomiphene may be discussed in selected men when stimulating the body’s own testosterone production and preserving fertility are priorities. Its use is off-label and requires an individualized evaluation and monitoring.
Anastrozole is not routinely needed with testosterone. It may be considered in selected circumstances after clinical assessment and appropriate laboratory testing, with attention to bone and metabolic health.
Nandrolone decanoate is an anabolic androgen and is not a standard testosterone-replacement formulation. When legally and medically appropriate, it may be considered separately for limited indications under close medical supervision.
Oxandrolone is an oral anabolic steroid, not routine TRT. Its FDA-approved indications are limited, and potential liver, lipid, hormonal, and cardiovascular risks require careful selection and monitoring when prescribed.
External testosterone can markedly suppress sperm production and, in some men, reduce the sperm count to zero (azoospermia). This can occur within months, but it is not accurate to promise that every patient will become azoospermic after six months. The degree and timing of suppression vary.
Men who may want biological children should discuss a baseline semen analysis and sperm cryopreservation before starting treatment. Our team can help arrange fertility testing and coordinate sperm banking with an andrology or fertility laboratory when appropriate.
If fertility becomes a priority after testosterone use, a clinician may recommend stopping testosterone and consider specialist-directed therapy intended to stimulate the body’s own hormone and sperm production. Recovery often takes months and may take a year or longer. Neither sperm recovery nor pregnancy can be guaranteed.

Medical testosterone treatment for confirmed deficiency is different from nonmedical anabolic-steroid misuse. Benefits, side effects, and risks depend on the patient, medication, dose, treatment goal, and quality of follow-up.
Testosterone prescribed to restore physiologic levels is not equivalent to supraphysiologic anabolic-steroid misuse. Mood changes can occur and should be reported, but extreme aggression is not an expected outcome of properly monitored replacement therapy.
Current evidence has not established that testosterone therapy causes prostate cancer. Baseline risk assessment and appropriate PSA monitoring remain important, especially in men over 40 or those with prostate-related risk factors.
Testosterone does not inevitably worsen urinary symptoms, but prostate size, PSA, urinary changes, and severe lower urinary-tract symptoms should be evaluated and monitored. New or worsening symptoms require medical review.
Starting treatment does not create an automatic lifetime obligation. Some men with potentially reversible contributors to suboptimal testosterone may improve after addressing sleep, weight, medications, illness, or other causes. Selected patients may be candidates for treatments that stimulate natural hormone production, but these are not the same as unsupervised “post-cycle therapy.” If an underlying deficiency persists, testosterone levels and symptoms may return after treatment is stopped. Any transition should be medically supervised.
Outcomes vary. Treatment decisions should balance possible symptom improvement against individual risks, contraindications, fertility goals, and the need for continued monitoring.
These laboratory markers are monitored because testosterone therapy can affect prostate-related testing and red-blood-cell production. A change does not automatically establish a new diagnosis, but it should be evaluated rather than ignored.
Testosterone can increase PSA in some patients. Baseline risk assessment and follow-up should reflect age, symptoms, family history, individual risk, and shared decision-making. A confirmed or concerning PSA change may require repeat testing, treatment adjustment, or urologic evaluation.
Testosterone can raise hemoglobin and hematocrit. Smoking, untreated or severe sleep apnea, lung disease, dehydration, and other causes of low oxygen or elevated hematocrit deserve special attention before and during treatment.
